Using MS Forms within Teams

I'm attempting to make a new quick poll within Teams using MS Forms, it all seems to be going smoothly until I click to send the poll to the chat,

At this point, it double posts the poll to the chat, doesn't let anybody respond to it and doesn't show my own vote.

I attempted a few other things to see how far the issue went:

-I tried to answer a quick poll from another user - the error message "Unable to reach app. Please try again" was displayed.

-I tried to create a poll using the bot @Forms Question? Option 1, Option 2. Function - the error message "Sorry, my bot code is having an issue" was displayed.

Has anybody had a similar issue and managed to solve this problem?

I've tried uninstalling forms, then reinstalling it, I also uninstalled Teams and reinstalled it to no avail.

Hi Seven,

Thanks for the reply, I updated my Teams to the most recent version and that seems to have fixed the issue,

There are still a few minor issues with using Forms but for the most part that seems to be fine!

    Thanks for your reply.

    From your updated information, your Teams may not be up to date. So please update your Teams to the latest version by clicking your profile picture at the top of the app and then selecting Check for updates. Since it cannot work on Teams Web App either, I suggest you restart both PC and Teams to check whether these three issues can be solved.

    If the issues still persist, to better help you, please kindly offer the following information to me:

    1.  Since only you have the issue, please try sign in your account on other members’ PC to check whether it is related to your account.

    2.  If convenient, please collect the Debug logs and send the log to me in private message so that I help you check the logs.

    No other members of my office have the same issue, it doesn't work on the web app either,

    My version number is 1.2.00.34161 (64 bit),

    Hello Thomas,

    As a temporary workaround, I suggest you can add a Forms tab so you can create a new form and here is an article for your reference: Create, edit, and collaborate on a form in Microsoft Teams.

    Besides, please also check if you have the same issue when signing into Teams Web App.

    If the issue still persists, please provide the following information to me for further assistance and I will consult others:

    1.  Please provide the version of your Teams so that I can do the same test on my side. To get the version, please click your avatar and select About > Version.

    2.  Whether other members in you team have the same issue?

    I will reply to you once I get any updates.
