We are using the SAC with Windows 10 1909. If the deferral period is set in intune to it's maximum 365 days, does that mean it won't try forcing an update until 18+12months after 1909 release date?

Or does the deferral period over-ride the SAC 18 months?

First note that there is no such thing as SAC. The last Windows version that had SAC and SAC-T was 1809. All versions after 1809 are essentially SAC with no SAC-T involved.

    On deferrals specifically, from https://learn.microsoft.com/en-us/windows/deployment/update/waas-manage-updates-wufb#offering:

Deferrals work by allowing you to specify the number of days after an update is released before it is offered to a device.

Deferrals have nothing to do with the supported lifetime of a feature update and this remains unchanged. Spring feature updates are supported for 18 months for all SKUs. Fall updates are supported for 18 months for non-enterprise, non-education SKUs and 30 months for enterprise and education SKUs.
