Outlooklive.com Possible Hack

Today my Outlooklive.com mail account refreshed by itself and the whole program had a different weird format. All my mail and history was there. It had a different URL. I signed out and in and all went back to normal. I went through everything and nothing looked suspicious. No unknown successful log-ins. No other incidents in other accounts. However, in outlook settings, general, mobile devices, there is a device listed under Universal Outlook that I do not recognize that is listed as syncing. When I delete it, it comes back on a refresh. Quite concerned here and I'm looking for an explanation besides a hack.

    Under my devices in my Outlook Account security, there are no unrecognizable devices. My account activity does not show any unrecognizable log ins as well. It's just that in account settings, general, mobile devices, there is an unrecognizable device under Universal Outlook that I can't seem to delete. It deletes, but comes back in time.
