Outlook for Mac adding attachments issues

I am using Outlook for Mac and lately i experience some problems when trying to add attachments.

For some reason, after the attachment has been properly added and the attachment icon appears, it's both gone on the moment i click the Send button! the document just disappear!

This problem is not consistent, and it's have been working properly before.
