Steps you need to follow to resolve the problem are
#1 - Open the CSV file in Excel
#2 - Deleted the first <blank> column (col CK is memory serves be correctly)
#3 - Save the corrected worksheet as a CSV file
#4 - Import the newly saved CSV file
