Email Hacked with Microsoft Authenticator

My email account is hosted my microsoft and I use Microsoft Authenticator. Today Authenticator popped up as if i was logging in to email, but i wasnt, so i declined. I then went in and checked history and found that my account had been accessed at least a dozen times successfully over the past week, sometimes only minutes apart. I changed my password and it stopped. I have a second account hosted by microsoft and the login history shows a similar amount of attempts recently but they all failed. I know they are not mine as they are all from an out of town windows computer and i run OSX. How did the hackers get around the Authenticator?? How do I stop it???

They're coming from different IP addresses and from Windows machines vs my Mac. There was 13 successful logins within an hour this morning, all from Windows, per the Authenticator history. I changed the password, there was one more attempt that failed and then they stopped.

It was asking you for permission to log in. Your answer is No so no one successfully logged into your account.

Those must be your IP address(es). Are you sure they aren't your IPs?
