Microsoft authenticator no longer asks for permission to log into my account. How can I fix this?

Microsoft Authenticator stopped asking for permissions when I log in. It just started happening on its own. I thought it was just on my xbox one s but it's on any device. I thought restarting the devices would help but no. I thought the app just needed updating. Under settings it says 2fa is still on but I receive no requests and can log into my accounts unhindered. I assume that means others can do the same. Whay can I do? Is this happening to anyone else?
