For a microsoft access project file, we have a form with add button to add records. For one particular record and user this add button disappeared. For others it remains visible. There is no custom code to hide the button. There is no user specific code. Why would this add button disappear?

If you tell me there is no code in the form or control, no custom security in place, etc.  then I would start looking at one of two things: (1) Office Install and (2) OS Install.

I would try a repair of the Office Install and make sure all the updates/service packs/... are all in place (check with your other user setup as a point of comparison - do they have the same verision with the same updates)

I would do the same for the OS.  Are they all running the same OS, Do they all have the same updates in place...
