Okay, on the custom install for Access did you make sure all features for Access AND the Office Tools and (I can't remember the other option but it has to do with shared Office stuff) were selected and set for RUN ALL FROM MY COMPUTER?

If you did do all of that, you might try going to Control Panel > Add/Remove Programs (or Programs and Features if on Windows Vista or Windows 7) and right click on the MIcrosoft Office or Access depending on which shows and select CHANGE (if Repair isn't available) and when it comes up you should be able to select Repair My Installation.

Give that a try and see if that will help.

When you installed Access (or Office) did you just use the default installation or did you click on Custom Install?

Well, first thing - don't use the lookup wizard to begin with.   You shouldn't be using lookups at table level.    See here for why:

    http://www.mvps.org/access/lookupfields.htm

You use lookups from forms (mostly combo boxes) to select the item from the lookup table and store the ID in the other table.
