Insert a Macro when using Checkboxes in Word 10

Good Morning,

I hope all is well.  I have been working on a form in Word 10 and I have evaluated

a couple of tutorial videos and I still can not solve my Macro problem.

I have created a form within Word 10 similar to a survey.  I have place Yes, No,

Unknown and NA checkboxes within the form. 

I would like the values of "No"and "Unknown" to have the value of 5 points when

checked, ultimately outlining the final score at the bottom of the form.

Any guidance that anyone provides is greatly appreciated.

    Good Evening,

    I thank you for your response.  The 'No' and 'Unknown' answers would have the 5 point value; for each question (20 questions in total (100 points possible)). 

    I inserted Active X Control checkboxes into the document and saved the document as a Word Macro - Enabled Templet.

    As the 'No' and 'Unknown' checkboxes are 'checked' I would like for the sum of all to be outlined

    in the 'Total' box at the bottom of the form.  Below is a shell of the form that I created.

    Any guidance you provide is greatly appreciated.

    What type of controls (check boxes) have you used on the form - Content Controls, Legacy FormFields or ActiveX check boxes?

    I assume that it is for a Yes that a value 5 points is applicable.
