Hide O365 FORMS statement

I want to use O365 FORMS for a simple survey, and I would like to hide this statement "Hi So-And-So, when you submit this form, the owner will be able to see your name and email address." from the person taking the survey. How ?

There is no option to achieve this.

If we untick "Record name", the "One response per person" will also be unavailable. Given this situation, we welcome you to submit feedback via Forms Uservoice.

    Hi OoiMeng,

    When you create the form, click the ... at the upper-right corner of the page > "Settings" > Untick "Record name". Click the "Share" button to share the new link with others, and the users who will response to this form will not see the "Hi username, when you submit this form, the owner will be able to see your name and email address." at the top.

    Thank you very much for your respond.

    I tried, but if I untick, the same person can attempt many times!

    My requirement is participants can only attempt one time only.

    How?
