Microsoft Forms Bug

I'm having an issue with a Microsoft Form I created several months ago. We have a question that involves a date ("Date of Visit") and every time we try to click on a date, the form either crashes or it jumps back to a random date in 2004 or something. Is this an issue with the form itself (I tried taking out the question and adding it back in again) or ...?

Hi Katherine,

You can try the following methods to check the outcome:

  1. Change into other browsers or open browser in Private mode to check it. For IE & Chrome, click Ctrl + Shift + P. For Chrome, click Ctrl + Shift + N.
  2. Create a new form and add the date question.
  3. Restart computer or change into a different environment, then click the date again.

If the issue persists, please let us know the following information so we can assist you further:

  1. The screenshot of the form crashes and jumps back to a random date.
  2. The detailed steps you create the question and involves a date.
  3. Check if the issue happens to other forms and Office 365 account.
