Creating questionnaire in SharePoint online

We are in need of taking a manual regulatory questionnaire and putting it online in our SharePoint environment.  Each questionnaire needs to be tied to the person completing it and NOT be anonymous.  Can I use Microsoft Forms for this or is that only for survey use?  And if I can use Forms, can that form then be used within a work flow?  Just trying to get a starting direction for this project. And I also need a system where we can inquire and extract data for future lookups and use.

    Msft Forms is deceptively named and looks in actuality like just an iframed spreadsheet - very amateurish and unprofessional. Depending on the complexity of the questionnaire,  you may be able to use a custom list and its associated form. I'd look into that before wasting time on infopath.

    Hi Guyinkalamazoo3,

    Microsoft Forms can’t be used in a site in SharePoint Online and work with a workflow. Based on your requirement, you need to use InfoPath 2013 to customize a form and make it work with a customized workflow such as an approval workflow. However, you can design the workflow as you like.

    Given the situation, I suggest you post a new thread on our TechNet forum-SharePoint 2013-Using SharePoint Designer, InfoPath and Other Customizations for better help.

    Thanks for the information and I will post this question to that other forum you mentioned.  The reason I wanted to avoid InfoPath is I heard it is going away soon.

    Yep, that is what I am planning on doing now.  I can see where MS Forms can be used as we do a few surveys here at work, but yeah, not for a formal questionnaire.
