Timesheet entry form

I am trying to create a form which managers can use to enter the daily time for employees.  The data gathered in the form would feed into an excel spreadsheet.

I have created the excel spreadsheet and the form that will be used to enter data.  The only question/problem that still has to be overcome is that for a single employee there could be a need to enter multiple cost codes and related hours.  My form looks like this right now.

  1. Date - Required / Set up so that calendar pops up
  2. Foreman/Manager - Required / set up with drop down with names
  3. Employee Name/# - Required / set up with drop down with names (equipment allocation is one of the selections under this - see below)
  4. Project #/Name - Required / set up as text response
  5. Phase/Cost Code - Required / set up as text response
  6. Time Worked - Required / set up as text response
  7. Equipment GPS - Set up with yes/no option answer
  8. Equipment #/Name - Set up as text response
  9. Equipment Hours

The questions/problem I have is that a single employee could have multiple phase/cost codes and resulting time in a day (question 5 and 6).  Is there a way to create a table or grid that would allow for this to be input (up to 5 phase cost codes.)  The same problem/issue is with equipment (question 7, 8, and 9). 

The issue is also how this would feed as separate line items into the Excel spreadsheet.  Thanks in advance.
