Formatting a questionnaire in Word

I need help on how to format a Word document suitable for a questionnaire. I have a grid with each question inside. I want another person to type their answer without the grid moving down. So each page is formatted with the individual questions, which a person completes, without disturbing the formatting of the next pages.

I hope that is clear and somebody can help me... the following is an example.

****
****

Do you Evaluate your service delivery and acknowledge its Social Impact?

If YES, How?

If NO, what holds you back?
Have you contracted with (or considering contracting)
an external provider to research and evaluate your company’s social impact? If NO, please explain way

****

****
****

How much might you consider spending(do you spend) annually on research, evaluation and reporting to demonstrate your social impact?

****

Do you budget or raise funds to professionally evaluate your service delivery?

****

****

The grid you refer to should be a Table with fixed-size cells. You can use formfields for the responses and protect the document for forms.

See if this what you need: Questionnaire

Note I have used Times New Roman if the font does suit do select all and change to the desired font.

Note: select the font name, not a Particular style.

While there are lots of suggestionsabout how to make questionnaires in Microsoft Word, you might give Excel's survey feature a peek.

    Go to OneDrive.com

Click New and then choose Excel Survey to get started
