In what way does the formatting change when you re-open the document?

Hello everyone. I'm running into a serious problem. I'm saving a Word doc as normal, yet when I reopen it, and any other Word doc, the format is completely changed. I'm working on research papers for school, and formatting the doc in required APA format. When I open the doc, I click on File (top left), then click options, then under SAVE, it states to save file in this format: "Word document (*.docx). I've not changed this at all. Under the Advanced tab, the box "keep track of formatting" is checked.
Back to the SAVE tab, is the format files are saved in needing to be changed from "word document (*.docx) to something else in the drop down menu. My operating system is MS Word 2013.
I'm at a loss for this as I've never run into this problem. Having to reformat is getting very old, very quick.

I'd suggest trying to repair the installation.
