From your description and the error message, it seems the Microsoft Forms page can’t show up well. To check if the issue is related to the Internet browser, please use other Internet browsers with incognito window to see if there’s any difference.
If you are using IE, you can also follow the steps to clear cache/reset browser to see the result:
- Clear cache: Internet Explorer>Settings>Safety>Delete browsing history>Make sure to uncheck Preserve Favorites website data and check both Temporary Internet Files and Cookies then click Delete.
- Reset browser: Internet>Toos>Internet options>Advanced>Reset to reset the browser
If the issue happens to every Internet browsers, to narrow down the issue, please provide us the information below:
- Please provide the detailed reproduce steps so we can know more clearly about how the error message occurs. For example, does the issue happen immediately when you sign in to Microsoft Forms? Or does the issue occur when you select a form to edit? Or does the issue occur when you want to response to a form?
- Does the issue happen to all users in your organization?
- When did the issue occur?
