Implement conditional logic in word template (office business 365 mac)

Does anyone know if there is a paid service one can use to implement some conditional logic into a word template I'm working on? 

These forums are helpful, however, I still have not been able to implement some conditional logic properly, such as showing/hiding fields, tables and cells based on previous answers; in specified tables, a limited number of table rows may be added if necessary; and appearance of conditional notes based on answers.  

I'm using office business 365 on mac to create a professional, on-brand, formatted report of a culmination of a long, complex workflow that is sold to clients. 

Survey tools (e.g. Survey Gizmo, Qualtrics) are great for branching, etc. but typically the completed report that can be downloaded as xls, pdf, or doc (depending on the service) are not professional looking; thus you're still stuck creating the logic in a formatted, on-brand template in word (or other service, but not sure which).

I do "for hire" custom template solutions.

You can use the email address in my signature line to contact me. Make sure to include a Subject in the note, otherwise our Spam filters will block it.
