Pivot table fields disappear after refreshing

I have a table of dates that feeds into a Pivot Table. However, every time I try to refresh it removes all the fields that I had originally chosen. 

I've turned on the option to keep cell formatting after refresh  and it still changes. I've also tried to change the data source with hopes that it would update, but nothing happens. 

Please help.

    Hi Nicole,

    Have you checked the suggestions of Vijay's and Jim's? Do Pivot table fields still disappear after refreshing?

    If you still need further assistance, could you please confirm the below information?

    1. If you create a new workbook with simple data and corresponding similar pivot table? Does the issue occur?
    2. Test result of safe mode.
    3. Your current Excel version (Excel > About Microsoft Excel).

    This was a bug in a build that has long since been fixed.

    Please go to the Excel menu and choose About Excel. You should have version 16.17 or 16.16.2

    If you have any other version, I am pretty sure installing updates will fix this.

    INSTALL UPDATES:

    I think the first troubleshooting step to take is: Ensure your Mac and Office have the latest updates. Here’s how to install updates:

    Make sure your Mac has a working internet connection, then:

    1. Update Mac OS X. Open the App store and click on the Updates button. Install the updates.
    2. After, in any Office application go to the Help menu and choose Check for Updates to launch the AutoUpdate app. Let AutoUpdate install updates and then restart your Mac.

    If you use Antidote, Webex, Silverlight, EndNote, Zotero or other add-ins there is a potential for crashes unless you also update your add-ins to the latest update. Install updates for these programs if you have them.

    Sometimes, add-ins can cause problems. To determine, if add-in is a problem start your application in safe mode and see if problem has gone away.

    To start in Safe Mode -

    Hold CTRL key and click on application icon and don't release the CTRL key till it asks you for Safe mode confirmation.

    If yes, then start application normally and disable add-ins one by one and start application again every time you disable an add-in to determine the culprit add-in.

    To disable add-in > File > Options > Add-ins > Down below you have Manage: Com Add-in : Press Go > Untick the Add-ins to disable and tick to enable them
