Microsoft Forms: missing Submission data

Hi,

I run a form my organisation for staff members to submit inquiries to management. So far it has worked well but I have ran into a situation where one user submitted 2 separate submissions, and neither have been tracked. Microsoft Forms sent him a confirmation email both times, but both the dashboard and the exported excel spreadsheet do not have a record of his completed submission. 

I also have some flows linked to the above form, and they did not track his submission either. 

Can somebody please help?

    Have you tried sending response using other users? Did you receive the response?

    Thanks for your response.

    I gave the user a different form to fill out and it worked (I received the notification and the submission details). 

    Currently the form is set to "only people in my organisation can respond", and from the auto-responder email that he received, it seems that he did send it from his company email/account.

    Hi,

    Does the issue only happen on the specific user? If other users can submit response fine, the root cause is more likely his account. 

    If you have set your forms to need sign in, better to make sure he has sign in with his work account in your organization then submit again. If you are the admin of Office 365, you can test his account on your side. 

    If the issue still persists, create a new form and see if he can use his account to submit response for it.

    Thanks for your response. I viewed your link, but unfortunately the information i am after is missing from the exported excel spreadsheet. My company uses Office 365 E3 so i dont have the "Sync all responses" option. Is there any other reason why the responses may be missing?
