Microsoft form I created doesn't show up in MS Teams

Question

I created a Microsoft form for the channel. It's usable. However, the form does not show up in MS Teams. The only way I can access the form to edit it or view responses it is by going to my browser and opening it their. Also the only way for me is to share my form regularly via link but not directly via MS Teams. Why is the form not showing on my list of forms? All I can do in MS Teams is create a new form from scratch. I have a business account with latest updates and I had tried to reinstall both extension and app. MS Teams connected to cloud and can be open in browser.

NB: the form is created in related group from MS Teams as described in your instruction

We appreciate that you posted back and confirmed the scenario with us.

We have checked your description again, and we found that you went to Microsoft group form and created a new form, then you tried adding it to Microsoft Teams.

If my understanding is right, according to the screenshot you provided, it seems that you went to Microsoft Teams Channel > New conversation, and clicked Forms icon, then you cannot select the form from Microsoft Forms.

If so, and if you want to add forms from Microsoft Form, you can go to click Add a tab inside Channel, as shown below:

Then you can add Form tab, then you can select Add an existing form, as shown below:

You can refer to the steps above to check if you can add exiting forms.

For the screenshot you provided, the way you was trying is to add a quiz, so you may not use that way to add exiting form.

If the scenario above is not consistent with yours, you can also post back and point that.

It's opposite. I went to Forms Web App, created form and want to add my form from it to MS Teams channel.

Here this form in web app of MS Forms:

When I try to add it to MS teams:

Your instructions says I should be able to choose it from pick-list. But I don't have this option.

Your questions:

From your description, it seems that you have created a form from Microsoft Teams Channel, however, this form doesn’t shown in Microsoft Forms Home page list.

May I know if my understanding is right? If so, generally, if you create a form in Microsoft Teams Channel, this form will not appear under My Forms, because it is created insider a team, you can go to Microsoft Form Home page > go to My Groups and select the team you created form, then you will see the form you created in Microsoft Team Channel.

We have also tested it on our side, we went to Microsoft Teams named class test and added a new tab, created a new form(new), then we went to Microsoft Forms Home page > click class test and we can see the form we created, as shown below:

If the scenario above is not consistent with yours, to better understand your situation, could you provide the information below?

1. You mentioned “Also the only way for me is to share my form regularly via link but not directly via MS Teams”, does it mean that you cannot share it via Microsoft Teams? Could you provide more details about it?
2. You mentioned “Why is the form not showing on my list of forms?”, does it mean that you cannot see this form in Microsoft Forms Home page?
3. Could you provide the detailed steps about how you create form in Microsoft Teams? So we can test again on our side.
4. You mentioned “I had tried to reinstall both extension and app”, may I know which app you have installed?
