Igor,

I am not the person who wrote this request but I actually am looking for something similar. We are in need of a way to have users part of a channel submit issues and allow us to track them.

I have chosen to go the route of Issue tracker vs Issue reporting / planner simply because tracker has a cleaner UI for the admin side to keep track.

Our goal is to have a way for users to submit issues they may have and have them add to the tracker. My first thought was a form. Is there a way to take form data and fill in the fields above into tracker / create a new issue?

Or if there is a way to take user input from issue reporting and add it to the issue tracker (not planner).

Let me know if you can think of anything.

Hi,

We have split your concern as a new thread to make sure we can offer the best support for your own demand.

So based on the detailed description, it seems like your demand is to get a similar issue tracker so that users can submit their issue including description and other details, which makes you think of a form to better collect the issue information. Feel free to let me know if I made any misunderstanding.

Generally, we can add the Microsoft List app into a Teams group and select the Issue Tracker template, which will look like this.

When users want to submit a new issue, click the +New button and it will ask for these information.

So kindly check if the above features can meet the requirements in your organization. If you still need Microsoft Form to get the issue details, then as there has no out-of-box connection between the Forms and Issue Tracker in Teams, it needs other flows to transfer/sync the data from MS Form to MS Lists.
