Microsoft Form Disappeared

I created a microsoft form to conduct a survey and sent out the link (which is still accessible) but the form is entirely gone when I log into forms. I followed numerous threads about the issue. I tried using different browsers and I tried using DesignPage.aspx#FormId. Using DesignPage.aspx#FormId brought up an error that collaboration was turned off by IT. I am IT and I didn't turn it off.
The link for the survey still functions...so where is the form so I can collect the responses?

    Hi Matthew,

Thanks for posting in the community.

According to your description, I assume the error message is “Sorry, something went wrong- IT policy has disabled the collaboration feature”. If so, please access Microsoft 365 admin center> Settings> Org settings> under Services, click Microsoft Forms and check the settings. Please turn on these settings. After that, access the form link and check again.

For more details, please view this reference: Administrator settings for Microsoft Forms

If the settings have already been enabled or you still can’t access the Edit view of these forms, the issue may be caused by the backend. I would like to suggest you create a support ticket in the Admin Center> Support> New service request. That support team there will have the correct channel to help you investigate more and find what exactly reason has caused this issue.

    Appreciate your understanding and patient.

    Regards,

    George
