word - macro, vba code for Heading 4, continued

For example, I have a heading 4 map title that is called "Instructions" in Arial 16 pt bold and the content of that map title goes on for several pages. I would like to be able to have at the bottom of each page a statement that is 'align text right' that says "Continued on next page" 10pt italics then a page break; and at the top of the following page for the map title to continue by saying "Instructions, Continued" with "Instructions and the comma" being in Arial 16pt. bold and "Continued" being in Arial 12pt. regular. I know nothing about VBA, code, or writing macros. Any help is greatly appreciated. Thank you.
