Repeating Content Control Checkboxes

I have a Word Form (in O365) that we use during a process that passes through three different groups of users as part of the process. I have an area at the top of the first page that contains content controls using checkboxes and two text box content controls. The goal is to have the first user group fill in the area at the top of the first page and have it repeat at the top of the follow-on pages. See image below.

Once the form is protected, I can get the plain text boxes to repeat as discussed in other posts to this forum. However, I cannot get the checkboxes from the first page to repeat in the same location on the follow-on pages. What am I not doing or am doing incorrectly?

    I would be using the StyleRef Field to get the page-specific information into my header as well. You do not need all those section breaks!

    Top of follow on pages = Header with Different First Page checked.

    You can use a StyleRef Field in the Header.

    You can map the checkboxes if you want to use elsewhere than the top of the page, especially if you want to be able to change them all from a different location.

    Mr, Kenyon...thank you for the prompt reply. I have one question to make sure I'm understanding what you are recommending.

    I am already using headers with section breaks between each section as each header has some different text in it. For example page 1 says Food Control Area following by Protective Action Recommendation. Page 2 says Food Control Area followed by Protective Action Decision and page 3 says Food Control Area followed by Protective Action Decision Rationale. Can I follow your recommended fix and still keep the current header text as is or would your fix replace the current header text on pages 2 and 3? Any alternative suggestions?
