Microsoft Form Multiple type in one question

Hello

I would like to have a question with option "yes" and "no". I would also like to add at the bottom of the question if they select "no" to a text box for them to enter in information of why they said no. Is there a way to do this.

    Dear SS (VA),

    Good day! Thank you for posting in Microsoft Community. We are happy to help you.

    As I understand you want to create a form with a Yes/No question, and in case respondent answers “No”, form opens a question where respondent must reply to additional question.

    If my understanding is correct, then you can achieve it with forms branching:

    • Open Microsoft Forms and create a form.
    • Add Yes/No question to form using Choice type.
    • Add a question where respondent must fill a reason why they answer “No” next.
    • Select Yes/No question and click More settings for question (…) > Add branching

    • In the branching menu click on a drop-down arrow next to “Yes” answer and select a question that goes after “Why No?” question.

    With these settings, when someone would respond to form and click “Yes” Forms would skip additional “Why?” question.
