Based on your description, we do understand you want to add the form questionnaire link to your webpage.
Per our search, some users mentioned that they can copy the embedded script and literally paste into the html file to have form shown on the webpage. Please refer to the link for how to embed a form in a webpage: Embed a survey in a webpage | Microsoft Learn
If you look at the embed code, you copy from Microsoft forms you can delete a part of it to leave only the form.
Instead of leaving the entire embed code in the box delete the rest of the code following the URL ending in embed=true" to check the outcome.
