Microsoft Forms advanced branching using checkboxes

Hello Office365 wizards,

I am currently brainstorming / building a Form that hopefully will be a tool used to assess crisis management events for our organization. My hope is to have some checkboxes that represent categories that may be relevant to an event that when selected, will populate the form with additional sections/questions that are specific to that category.

For example:

If the checkbox for "Security" is selected, I would want an section to automatically populate on the form to have a handful of questions or options specifically related to the "Security" category. On the flip side, I would not want the "Security" questions to show up on the form if the checkbox wasn't selected.

Right now, I have about 13 categories that could be relevant to an event and each category may have 5+ questions per category so the form could get rather long and tedious which is something I am trying to avoid. In addition, I would want the form to be quick to fill out (10< minutes).

From this form, I am hoping to create a Flow to start a Teams call based on the information filled out in the form.

Looking forward to any feedback or assistance!

    Dear Tim,

Good day! Thank you for posting your query in our community.

For your requirement with advanced branching in Forms, you can try the following steps:

    1. Put the main question in Section 1, create multiple sections with each option of the main question as the section title, then insert questions in each related section. Image
    2. In Section 1 with the main question, click … and select Add branching. Then select the corresponding section in the box after Go to for each option of the main question. Image
    3. If you want to create other questions like the main question in Section 1, repeat the above steps, and select the next section in the box after Go to for each option of the section.
    4. If you don't want, you need to select "End of the form" in the box after Go to for each option of the section to prevent users from seeing the "Tech" questions on the form if the checkbox wasn't selected. Image

Additionally, for your other requirement "create a flow", you may need to use Microsoft Power Automate, so my suggestion is to post on the dedicated Microsoft Power Automate Community, the members and engineers in this community are more experienced in this area. You will get a more detailed and professional answer to your problem there.

If my description is not clear to you, please feel free to post back.

    Sincerely,
