I have resolved the issue. For the benefit of others here are are answers to your questions:
- Deleting the cache alone did not resolve the issue
- The issue did not show up in Microsoft Lists
- The issue was isolated to my user account on this computer. I did not see this issue when logged into a different machine.
- The problem was isolated to this list only.
To solve the issue, I removed the cache you spoke of in your reply, cleared my browser cache, and finally restarted the machine.
