Text alignment problem when indexing

When I'm indexing a large document and I mark an entry, the index sometimes aligns the whole paragraph to the right. I want the formatting to remain 'justified' but it changes every time. How do I do this? Is there a way to stop the {XE index} infix from realigning my paragraphs?

I also have been seeing some big grey boxes appearing in the text when I mark entries for the index. This may be related to the fact that I converted this word document from a pdf using Acrobat?

Thanks,
