Issue with microsoft forms when exporting to excel - losing column question titles.

I have been using forms to gather feedback surveys from our patients and am coming across an issue with how the information is exported to excel.

the feedback outcomes / responses are all listed correctly, but the column titles / questions are the issue.

for all of the multiple-choice questions, the excel column showing their responses doesn't keep the question at the top, it lists a number (which doesn't actually correspond to anything).

the 'free answer' questions display the written question in the excel column.

I am trying to find a way to have all of the written questions displayed at the top of each column.

Is this doable?

Thank you!

Create a similar form with some dummy data and share us some screenshot of your issue. Then we may recreate your scenario.

    Hi Snow,

Thanks for getting back to me.

The question titles are definitely entered into the Question field, and as far as I can tell they have saved correctly.

I've exported multiple times and the issue is still ongoing.

How would you like me to share the form with you?

    Thank you!
    Dior

When you export your form data to Excel, the column headers should match the question titles in your form. If you're seeing numbers instead of the question titles for your multiple-choice questions, it's possible that the question titles were not entered correctly or were not saved properly.

To ensure that your question titles are saved correctly, please make sure to enter them in the "Question" field when creating your form. You can also try exporting your form data to Excel again to see if the issue has been resolved.

If issue persist, could you share us a your form to test on it?
