TOC split into two lines for one entry

I got TOC split into two lines (one line for heading and one line for the paragraph number) for one entry and the page number duplicates; not all entries are wrong, only a few.

Wrong one:

         use correct tools ................38

4.3.12.9 38

Correct one (what I want):

4.3.12.9 use correct tools ................38
