Issue with document upload in MS Forms

I currently have an internal system running for Approvals based on information inputted via a MS Forms document, but it won't allow any attachments to the relevant question.

It allows selection of the document you want to upload, gets about 80% uploaded then disappears.

I've tried

  • clearing cache/cookies
  • uploading a document via the account which the Form was created on and it allows upload, but doesn't allow from any other account within the organisation.

Any help would be much appreciated as this is hindering a few things that operate via this method.
