When running on Windows 10 on a personal machine, we place people in Monitoring mode.
In this mode, PDF's cannot be previewed from outlook unless the user is running FireFox, Edge and Chrome do not seem to work.
Is this by design or is this a bug / issue?

no controls exist that are browser dependant on Azure CA
The Azure CA would only be worth investigating if it didn't allow preview in Firefox, which it does, just not edge or chrome

By this, do you mean the issue only occurs when people are placed in "Monitoring mode"?
And by "outlook", you are referring to the Outlook on the web, right?
What's the version of your Exchange server?
Is there any error message when the PDF files failed to be previewed in Edge or Chrome?
To help narrow down the issue, I'd recommend testing with another account like an personal outlook.com account and see whether the issue can be reproduced.
Besides, it's suggested to upgrade Edge and Chrome to the latest version and see if there would be any improvement.

Yea, someone set up some policies/session controls in your tenant to block those.
You need to ask your admins about this. Looks like a Cloud App Security policy:
