Microsoft Forms Links and QR Codes Not Working

I created a Microsoft Form via my browser (google chrome) on Windows 10 Pro (work computer). I've tried generating a QR code through Forms, as well as using a free QR code generation website. I have also tried using the direct link (both full and shortened). In all instances, the QR code / link results in an endless loading wheel. Permissions are set to "anyone can respond." This is the link:

I'm having the same issue as above. it worked when I first tried it and took me to the form I created but when I tested in again after an edit I keep getting a continuous loading screen. Now that's happening every time I scan my code.

Same here - QR code accessible only once per device.

Can you advise how to fix it.

Had the same issue with the QR codes at my company, sounds stupid but if you clear your browser history cache and cookies it will work.
