Problems with inserting MS Forms into PowerPoint

Hi, I am trying to insert a number of MS Forms into a presentation but I'm having intermittent problems with them displaying

I am an a hybrid 365 tenant with a business licence using Microsoft 365

I can insert the form with no problem and I choose the 'Present for interactive meetings' option and initially I can see the form. However, when I open it next time, I see a page saying "This form doesn't exist". (Although sometimes it loads straight away). Other people viewing the PowerPoint sometimes see the forms but most often just see the "This form doesn't exist" message. This is happening on multiple forms in one presentation.

It's the fact that sometimes a form loads and other times not that's confusing me. If it failed every time I could look for a solution a bit more easily

Debugging gives me a few errors regarding cookies. The most concerning is:

Failed to load resource: the server responded with a status of 404 () /formapi/api/239f138...$expand=choices):1

Failed to load resource: the server responded with a status of 404 () /formapi/api/239f138...)localeResource:1

Looking at these api's the message is:

error

code: "701"

message: Required User login"

ms.fom.error.type. :"Expected Failure"

As extra information, the PowerPoint is stored in a Sharepoint document library and the forms have been moved to a group forms.

This has been tested with members of the SharePoint group (plus I'm a group Admin) so don't think it's an access issue.

I have also tried this with new presentations and new forms and I'm still getting the intermittent errors.

Inserting a Form as "Collect response offline" works absolutely fine everytime

Any ideas?
