According to your description, I suggest you check the List settings. In the list, Settings> List Settings> Form settings. Make sure that the "Use a custom form created in PowerApps" option is ticked.
- If not, please tick the option, click OK, and then check again.
- If so, some users mentioned that re-enabling the option solved the problem. So, I suggest you tick "Use the default SharePoint form", click OK, refresh the page, tick the "Use a custom form created in PowerApps" option again, and click OK. After that, please check again.
