Subject: Need Help Accessing Microsoft Forms

Hi everyone,

I'm having trouble logging into Microsoft Forms using my email address and am receiving an error message. I can't seem to find Microsoft Forms listed under the applications in Microsoft Azure > Enterprise Applications.

How can I gain access to Microsoft Forms? This is crucial because all my client forms and data are stored there, and I don't have a backup.

I've also tried the link provided here: Microsoft Forms Documentation, but it didn't resolve the issue.

Could someone please help me with this?

    Dear Carly De Leon,

    Thanks for writing back and updating with us. Apologies for the delay in the response.

    As per the further updates, I would like to let you know that the error message you are encountering when accessing the Form generally occurs when the Microsoft Forms app license is not assigned to your Office 365 account.

    So, I request you to connect with the IT admin of your organization and request them to assign the Microsoft Forms license to you, here are the steps for your IT admin to perform:

    Open the Microsoft 365 admin center> Active Users> Click on your username> License and apps> Scroll down and click on Apps> Scroll down to Microsoft Forms and enable it> Click on Save Changes.

    Once the IT admin of your organization has enabled the Microsoft Forms to your Office 365 account, please sign out from your Office 365 account and sign-in back to check the outcome.

    2. Do you have the same error message when you access the Microsoft Forms in the InPrivate or Incognito window of the browser?

    Yes

    Apologies for the inconvenience caused and please don’t worry, let’s work together on your concern and move towards a resolution path.

    I understand your concern and I would like to confirm whether you are having the Microsoft Forms license in your Office 365 subscription page:

    Open the browser such as Google Chrome> Click on the three dots at the top right> New Incognito window> Access https://portal.office.com > Enter Office 365 account details> Click on the profile picture at the top right (beside the question mark icon)> View Account> Scroll little down and click on Subscriptions> See whether you are having Microsoft Forms.

    If you are not having the Microsoft Forms, then please enable the Microsoft Forms in the Apps of your Office 365 account:

    Open the Office 365 admin center> Active Users> Click on the user name> License and apps> Scroll down and expand Apps> Scroll down and enable Microsoft Forms> Save it.

    Note: If you are not the IT admin of your Office 365 organization, please connect with the IT admin of your organization for performing the above steps.

    If the Microsoft Forms is enabled but not able to access to Microsoft Forms, I would like to confirm the following information which will help us to better understand and guide you further:

    1. Screenshot of the error message when trying to access to Microsoft Forms.
    2. Do you have the same error message when you access the Microsoft Forms in the InPrivate or Incognito window of the browser?
    3. Do other users in your team are having the same issue?
