Customize TOC Content with additional fields - not just page numbers

My client needs to for the document we are creating to have a TOC that has Headings, then revision number, then page number such as:

Section Revision Page

  1. heading text .......................................................... 2 ......................... 3 of 23
  2. heading text .......................................................... 4 ......................... 9 of 23

The revision number can be a bookmark or some other field. The document is legally binding in that it controls allowable behavior at the facility and individual "limitations" are specified in the numbered sections. They change form time to time and traceability is required. Word 365 is being used. Headings 1,2,3 will be used.

    There is no way this will work. Every time the TOC is updated, the added fields/text will be removed.

    2. Combining the Fields:
