Hello, KB_1231.
Welcome to Microsoft Community.
In Microsoft Forms, you can create a question with multiple sub-questions or answer options by using the "Likert" or "Choice" question types. Here’s how you can achieve a structure where multiple questions appear under one question, similar to a dropdown or grouped format:
**Method 1:**Using the Likert Question Type (Best for Grouped Questions)
- Open Microsoft Forms and create or open your form, here is the entry: Microsoft Forms | Surveys, Polls, and Quizzes
- Click Add New and select the "Likert" option (you might need to click the dropdown next to “Choice” to see this option).
- This will allow you to add multiple statements (sub-questions) on the left and multiple answer options (like "Agree," "Neutral," "Disagree") across the top.
- Customize the statements (questions) and answer options according to your needs.
Example:
- Statement 1: "What is the primary function of your role?"
- Statement 2: "Which software do you use most frequently?"
- Answer Options: "Managing Projects," "Sales," "Customer Support," etc.
- As shown here:

**Method 2:**Using the Choice Question Type (Best for Dropdown-like Questions)
If you want to have a dropdown-like question with multiple answers, you can also use the Choice question with multiple options:
- Add New and select "Choice" as the question type.
- Add the question text for your main question.
- Select the option for "Drop-down" (you'll see it below the options).
- Add the answer choices. If you need to allow multiple selections, please click the “Add New Question” option.

5. If you need to simulate multiple questions, you can create multiple **Choice** questions under a common theme.
**Method 3:**Grouping Questions
Although Microsoft Forms doesn’t support true hierarchical dropdowns, you can create a Section to group multiple questions together:
- Select Add New and then click the three-dot menu (...) and choose Section.
- Give the section a title that represents your main question.
- Add individual questions (such as Choice or Text question types) under this section to create a grouped question experience. Here is a test form, see:
