Anonymous form showing "When you submit this form, the owner will see your name and email address." message

I have created an anonymous form in Microsoft.

In the settings, we have selected 'only people from my organisation can respond' to ensure the form isn't shared externally.

We want the answers to be anonymous, but on the form, there is a message that says, "When you submit this form, the owner will see your name and email address."

What do we need to change to remove this message? And will it then be truly anonymous?

    I did a test here and when I did not check Record the name, the owner of the form was unable to see the user's name and email after submitting the reply:

    If the Record the name is checked, the email address and name will be displayed, you can refer to the screenshot for comparison:

    Therefore, I would like to confirm whether you check this option first. If you also do not check this option, can I further confirm whether this phenomenon occurs in a specific form? This can help me troubleshoot whether there is a problem with a specific form or a problem on the Forms server side, so as to further investigate this phenomenon for you.
