Power automate flow creating duplicate items in MS List after response submitted in MS Forms

I have a flow that is triggered when a response is submitted on a MS Form and creates an item in a MS List. This was working perfectly until suddenly started creating duplicate items in the list - I did not edit or amend the flow before this happened nor did I edit the MS Form.

I was amending the MS List new item form in Power Apps before I noticed it, but I'm not sure this had any impact (the form was already created in Power Apps, I was just changing a field).

In MS Forms, the response page shows one response per submission so doesn't look like a form issue. In Power Automate the flow runs with no issues and the run history shoes one run per instance.

I have created a new form and tried that with the flow and have also created a new flow, but nothing has fixed the issue.

I also tried turning concurrency control on with degree of parallelism turned to 1, but this also didn't help.

I don't want to add anything to my flow as it was running correctly until randomly today, so there is no fundamental issue there, something has just changed?

I have inserted a photo of the duplicates (they are identical), the flow and the MS List new item form in Power Apps.
