For resolving Forms access issue, please kindly refer the following document to check the options:
Turn off or turn on Microsoft Forms - Microsoft Forms Admin | Microsoft Learn
- Please kindly confirm whether you have assigned a license that includes Forms.
Microsoft 365 admin center - Users - Active users - choose the user - license and apps - confirm forms has been checked
If you already assigned license but still cannot access Forms, please check org settings is step 2.
- By default, Microsoft Forms is turned on for everyone in your organization.
Please kindly double confirm the following settings:
Microsoft Entra admin center - applications - enterprise applications - search for Microsoft Forms
Properties - enabled for users to sign-in - Yes
