Portal List not displaying entered data

Hi,

I have created a basic portal that captures information with a form, but when I list the form, the records display, but when I click view details on a record, the entered information does not display please advise or send me in the right direction, thank you. (attached screens)

    Can you clarify how this relates to Power Query? I'd suggest you post this in the community forum for whatever product you're using, or update the tag to point to the correct product.
