MS Auth App Not Displaying 2 Digit Code

We have an end user that is not receiving the 2 digit MFA code from her work machine to her iPhone within the Microsoft Authenticator app. It has worked in the past but quit working a few days ago (user's phone is not new).

Things that have been tried already but have proven unsuccessful are as follows.

  • MFA has been reset in Entra several times; completely removed to start fresh.  Even reset and left alone for several hours before trying again 
  • User was issued a TAP twice on two separate occasions.  Both times she entered the one time TAP but received a message that the password was incorrect both times
  • Checked her iPhone and workstation clock are in sync
  • Had her check to make sure that she did not have Do Not Disturb, disable notifications, etc set on her phone and they weren't.  
  • Had user uninstall the authenticator app from her phone and reinstall
  • User recently updated to the latest software update 26.1
  • Had user remove all work accounts from her auth app and readd under Work or School Account.  She can scan QR code but when the 2 digit comes up on the workstation, she is not prompted to enter that on her auth app.
  • Had her try the alternate method of using the 6 digit code from the auth app and enter that on her workstation, that did not work either.
  •  We had her restart her phone.
  • We had another user log in on this user's machine with their own credentials and authenticator app and they were prompted for the 2 digits.
