Append a tag and its value to resources not working - apply this policy doesn't work all I am getting is that all appliances are non-compliant and no updates on the tags

@David Harvie Welcome to Microsoft Q & A Community Forum. Append effect is used to add additional fields to the requested resource during creation or update. When you apply policy which has append effect, it reports the list of resources that are non-complaint but it doesn't modify the resources to make them compliant. Policy with append effect make sure that when new resources or existing resources are updated, then the tag value is added. Same information is documented here.

How do I do a modified effect I can't find an option to do that?

@David Harvie There is an existing built-in policy to modify tags of Azure resources which you can check out here. Kindly note this policy adds the specified tag and value when any resource missing this tag is created or updated. Existing resources can be remediated by triggering a remediation task. If the tag exists with a different value it will not be changed. Does not modify tags on resource groups.
