Can't register for Microsoft Hardware Developer Program

Based on various threads online, I was able to get to the registration page of Hardware Developer Program.

After filling the details, I am not able to move to the next page as 'Next' button is unresponsive. Without this, I am not able to upload my drivers for review.
In Partner Center, our organization is already Verified. There is no way to raise a support ticket with Microsoft.
Has anyone been able to go beyond this page? Please help me as our product release is getting delayed.
