My quota covers the number of cores.
I get the same result if I select any other region? i.e. UK West
An Azure relational database service.

Even for regions that support SQL Managed Instance zone redundancy, it may show up as not supporting it due to capacity limitations. Have you tried West Europe? Below is screenshot from create wizard with West Europe selected:

Hi, I tried in UK South and West and it wouldn't work. Works in the US regions fine.
I did a proof of concept in UK South a month ago ok.
My problem is all of our infrastructure is deployed in UK South and we need SQL Managed instance to be deployed there.
What are our options, how do I get a definitive answer as to resource constraints and how do we request resource?
You can submit a quota request and ask them to enable it for UK South your subscription. They may deny your request due to capacity limitations.
Please carefully follow instructions below to create a new Service and subscription limits (quotas) request to allow you to deploy zone redundant SQL Managed Instance in UK South.
1. Navigate to this link to start the process:
https://portal.azure.com/#create/Microsoft.Support
2. Enter quota in the box and click Go
3. Select Service and subscription limits (quotas) and click Next
4. Click Create a support request button, as shown
5. Select Issue type: Service and subscription limits (quotas), Subscription: <subscription that needs quota increase>, Quota type: Other Requests, click Next
6. On Additional details tab, enter description for your request, for example, that you need zone redundancy enabled for SQL Managed Instance in UK South
7. Finish filling out Additional details tab, click Next, review and click Create. You should receive an automated email within a few minutes.
In addition to above, I suggest sending private message and asking Microsoft External Staff Pilladi Padma Sai Manisha to create enablement request on your behalf.
