System displayed unexpected error message when, click on the submit button from first screen

On my first attempt, I entered all the required details along with the payment information and submitted the form, but I did not receive any confirmation and faced 404- Page not found error.

On my second attempt, I filled in all the details on the first screen, including address, ZIP code, state, country, and phone number, and then clicked the submit button. However, the system displayed an unexpected error.

Kindly assist in resolving this issue at the earliest, as I am unable to proceed with GH- 300 : Github copilot exam booking.
