Hardware Partner request blocked

I try to enroll into Hardware Partner program for driver signing. When I fill all the information and click "Accept and Continue" button I get this error:

"Microsoft runs on trust****. We engage in a rigorous set of evaluation and certification processes; as a result your request was blocked. If you require further information please reach out to Microsoft support with reference number: 715-123225 and transaction ID: 4f8c819b-f919-40ef-a480-933734e8a5d7."

I tried to reach the Support from Dashboard, it is impossible to create ticket, because I can't select the workspace.

I tried to create support request at http://aka.ms/hardwaresupport, but when I select "Enrollment" it will forward me to the same Dashboard support.

Is there any solution on how to solve this problem?
